Service Eligibility:

To qualify for our services, patients must be homebound and in the care of a physician, receiving services under a plan of care established and periodically reviewed by a physician, be in need of skilled nursing care, physical, speech, and occupational therapy on an intermittent basis.

We provide services to homebound patients with a wide range of diagnosis.
